Latest quarter
Insulet reported third-quarter revenue of $842.0 million, up 19.2% from $706.3 million a year earlier and above the $833.3 million consensus. Adjusted earnings of $1.99 per share also exceeded the $1.59 consensus. The company’s constant-currency revenue growth of 19.6% was above its own guidance range.
The filing attributed U.S. Omnipod growth to higher sales volume from an expanding customer base. International Omnipod revenue rose to $266.2 million, with higher volumes and, to a lesser extent, higher average selling prices as customers moved from Omnipod DASH to Omnipod 5. Drug Delivery revenue fell to $5.5 million as orders declined.
Gross margin rose to 75.1% from 72.2%; the filing attributed the increase primarily to manufacturing efficiencies, with a smaller benefit from a warranty accrual adjustment. Operating margin increased to 22.5% from 16.7%, while quarterly free cash flow fell to $66.4 million from $94.6 million.

What management said on the call
CEO Ashley McEvoy said the global customer base grew more than 20.0% from a year earlier, providing a recurring revenue base as customers purchase replacement Pods. CFO Flavia Pease said U.S. new customer starts were flat year over year, while the customer base grew in the high teens. Pease said U.S. pricing slipped slightly in the quarter but was flat for the year to date; internationally, volume and favorable price mix supported growth, with reserve adjustments also contributing.
McEvoy said Type 2 retention improved modestly after Insulet changed its sales incentives and sampling approach and added clinical retention specialists. She said the company had completed the U.S. release of Omnipod Discover, its data and engagement platform, in October. Pease said Insulet was not assuming additional financial benefits from the onboarding and support actions for the rest of the year.
On profitability, Pease attributed adjusted gross margin expansion to productivity gains across operations. She said adjusted operating margin also benefited from lower annual employee bonus accruals, while Insulet continued investing in its sales force, customer support and product pipeline. McEvoy said the company had submitted the final regulatory application for Omnipod 6 and continued to expect a 2027 launch.
Analysts pressed for more detail on the preliminary 2027 outlook, competition and Type 2 retention. McEvoy said Insulet expected to lose some share of new customer starts as competitors entered, but expected its customer-base share to expand. Pease said international growth would moderate as large country launches passed their anniversaries and the price-mix benefit diminished. She said the company would provide precise ranges in February, adding: “Not going to provide specifics until we got in February.”
For the fourth quarter, Pease said Insulet expected U.S. attrition and utilization to remain similar to third-quarter levels. McEvoy said the board had approved an increase in the share-repurchase authorization and the company expected to fund fourth-quarter repurchases with cash on hand.
Guidance
Insulet narrowed its full-year 2026 constant-currency revenue growth outlook to 21.0%–22.0%, the high end of its prior range. It also raised its international Omnipod constant-currency growth outlook to 32.0%–33.0% and increased its expected adjusted operating-margin expansion.
For 2027, Pease said Insulet now expects total-company growth modestly below its earlier mid-teens framework. She said the company expects to guide U.S. growth toward the lower end of its prior framework and international growth to moderate meaningfully from 2026.
