What it does
The Cigna Group is a global health company that sells pharmacy services and health benefits to employers, health plans, government programs and individuals. Founded in 1982 and headquartered in Bloomfield, it is led by chief executive Brian Evanko.
Its Evernorth Health Services business helps clients manage prescription drug benefits. It processes pharmacy claims, builds pharmacy networks, advises on benefit design and manages lists of covered medicines, known as formularies. Evernorth also operates home-delivery and specialty pharmacies, distributes specialty medicines to health care providers and offers virtual care and other clinical services. Clients can buy services individually or combine them.
Cigna Healthcare sells medical plans and related benefits to employers, other groups and individuals. It offers both insurance, where Cigna generally takes on the risk of covered medical costs, and administration for self-funded plans, where the employer or other plan sponsor funds claims. Its offerings also include dental, behavioral health and stop-loss coverage, which reimburses self-funded clients for claims above a specified threshold.
The businesses can work together: Cigna Healthcare clients can combine medical benefits with Evernorth pharmacy services. Cigna also offers health coverage internationally, including for multinational employers and people who live or work across borders.
How it makes money
Cigna earns pharmacy revenue from filling prescriptions and providing related services through Evernorth. Its pharmacy benefit business processes claims, administers retail networks and manages drug formularies and rebate arrangements for clients. It also earns fees and shares in value created by negotiating pharmacy, drug purchasing and manufacturer rebate arrangements. Revenue and income depend partly on prescription volumes, the types of drugs dispensed and contract terms.
Evernorth’s specialty business dispenses medicines for complex or rare conditions and distributes pharmaceuticals and supplies to providers. Its care services include virtual care, benefits management, behavioral health and health coaching. These services can be sold alongside pharmacy benefits or separately.
Cigna Healthcare receives premiums for insured plans and fees for administering self-funded plans. For self-funded arrangements, the plan sponsor pays claims, while Cigna charges for provider-network access, claims administration and other services. Those arrangements represented approximately 32% of 2025 segment revenues and 79% of Cigna Healthcare medical customers as of December 31, 2025.
For insured plans, Cigna sets premiums for a policy period and generally bears the risk if covered costs exceed those premiums. It retains the margin when costs are lower, subject to minimum medical loss ratio rules that can require refunds. Insured arrangements represented approximately 68% of 2025 segment revenues and 21% of medical customers as of December 31, 2025; the revenue figure includes Medicare Advantage before its sale.
In the third quarter of 2026, Cigna reported adjusted revenues of $73.8 billion. Prescription use, ongoing plan administration and insurance policy periods generate continuing business, although clients and contracts can change.
Customers and how it sells
Evernorth serves managed care organizations, insurers, employers, third-party administrators, union-sponsored plans, government health programs, providers and hospitals. It also serves most Cigna Healthcare clients. Patients receive prescriptions through contracted retail pharmacies and Evernorth’s home-delivery and specialty pharmacies.
A single pharmacy benefit client accounted for 19.0% of revenue in FY2025. Evernorth’s significant clients include Centene, Prime Therapeutics and the military’s TRICARE program. Cigna has renewed or extended contracts with its three largest Evernorth clients through the end of the decade.
Cigna Healthcare serves employers and other groups as well as individuals buying coverage. It distributes products through brokers and consultants, its own sales representatives, private exchanges and public health insurance exchanges. Its employer medical plans are offered nationwide.
Outside the United States, Cigna offers health coverage to multinational employers and globally mobile individuals, as well as local customers in markets including China, Singapore, Hong Kong, Spain, India and countries in the Middle East. Across the company, there were more than 185 million customer relationships in more than 30 markets and jurisdictions as of December 31, 2025. Cigna Healthcare had 18.5 million medical customers in the third quarter of 2026.
Suppliers and costs
Evernorth buys pharmaceuticals directly from manufacturers or through authorized wholesalers. It uses one wholesaler for approximately half of its pharmaceutical purchases, while maintaining contracts with other wholesalers as alternative sources. Generic drugs are generally purchased directly from manufacturers. Its home-delivery pharmacies, specialty pharmacies and specialty distributor hold inventories of medicines; they can generally obtain drugs not in inventory from a supplier within a reasonable time.
Drug acquisition and pharmacy services are major costs. Changes in prescription volumes, drug prices and the mix of branded, specialty, generic and biosimilar medicines affect Evernorth’s results. The company also negotiates with pharmacies, manufacturers and wholesalers to manage purchasing costs and rebates.
For Cigna Healthcare, medical costs depend on how much care customers use, the cost of each service and the mix of services. Other cash requirements include employee compensation and benefits, technology, facilities, taxes and debt service. In 2025, pharmacy and other service costs increased 18%, primarily reflecting higher prescription drug utilization from customer growth in Evernorth.

Competitive position
Cigna competes across pharmacy benefits, specialty pharmacy, health insurance and care services. It identifies competitor types rather than naming specific competitors. In pharmacy services, these include other pharmacy benefit managers, retail and specialty pharmacies, drug distributors and care providers. In health benefits, they include national insurers, local health plans, benefit administrators and international insurers.
Competition depends on pharmacy and provider network access, drug purchasing and rebate negotiations, care management, cost, technology and sales. Cigna Healthcare’s U.S. network had approximately 1.7 million physicians and over 6,000 hospitals as of December 31, 2025. Evernorth offers pharmacy benefits, specialty dispensing and care services together or separately, and its specialty pharmacy has access to limited-distribution medicines.
Our reading: The provider network supports Cigna’s competitive position because access to doctors and hospitals matters to plan clients, but it must continue to contract with providers on competitive terms. Our reading: Combining medical and pharmacy services gives Cigna opportunities to serve more needs of an existing client, although clients can also buy services separately and face competing providers in each category. Our reading: Specialty pharmacy access and the facilities needed to prepare complex medicines are sources of differentiation, but continued access to key drugs remains important.
Growth opportunities
Cigna is pursuing growth through specialty pharmacy and distribution, pharmacy benefit services and employer health benefits. Specialty services can grow through new clients, existing-client expansion and access to new treatments. Cigna estimates the specialty market represents a $480 billion addressable market.
Cigna is expanding capacity for limited-distribution medicines and is building a fifth clean room for preparing complex drugs. Its recently launched Evernorth Wholesale has begun fulfilling orders to its distribution centers. In Cigna Healthcare, HealthSense launched in September as a customer-engagement service initially focused on oncology; the company expects it to broaden to other conditions.
Cigna is also transitioning to a rebate-free pharmacy benefit model called Signature. The model is scheduled for Cigna Healthcare’s fully insured customers in 2027, with broader Evernorth adoption expected to ramp in 2028. The company is negotiating with drug manufacturers and network pharmacies as it builds the model. It expects investment and transition costs to affect Evernorth’s pre-tax adjusted income from operations in the short term.
Cigna expects its Accelerate to One modernization and productivity initiative to contribute $3 billion in gross savings by 2030. It plans to reinvest some of those savings in the business. The company has also announced an exit from the ACA exchange market, which it expects to create an approximately $200 million headwind in Cigna Healthcare in 2027.

Structural risks
-
Medical cost and pricing risk. Cigna must predict and price for health care costs. For most insured policies, it cannot adjust premiums for actual claims experience until the next policy period, leaving it exposed when costs exceed premiums.
-
Regulation and legal challenges. Pharmacy benefit practices, insurance, drug distribution and health data are subject to extensive regulation. Changes in laws or enforcement could affect business practices and results. Cigna is also subject to litigation alleging improper use of AI in claims evaluation.
-
Client and prescription volume. One pharmacy benefit client accounted for 19.0% of revenue in FY2025. Cigna reported declines in pharmacy benefit services prescriptions and customer counts during 2026, including reduced volumes from health plan clients.
-
Drug and supplier exposure. Drug acquisition costs, pricing trends and the availability of generic or biosimilar alternatives can change. Evernorth uses one wholesaler for approximately half of its pharmaceutical purchases, although it has contracts with alternatives.
-
Technology and data. Operations depend on the uninterrupted availability and integrity of information technology systems. Health information is subject to privacy and security requirements, and violations can lead to enforcement or claims.
-
Debt and acquired assets. Total indebtedness was approximately $31.5 billion as of December 31, 2025. Goodwill and other intangible assets were approximately $73.5 billion, representing 47% of total consolidated assets at that date; underperformance or future evaluations could lead to impairment charges.
Bottom line
Our reading
Cigna combines a pharmacy services business with a health benefits business, giving it several ways to serve the same clients. Our reading is that its provider networks, specialty capabilities and combined offerings support those relationships, while results remain exposed to large clients, medical and drug costs, regulation and the transition in pharmacy benefit pricing. The planned exit from ACA exchanges and the shift to Signature also make execution important.
