Latest quarter
Baxter reported third-quarter revenue of $3.0 billion, above the $2.9 billion consensus and up from $2.8 billion a year earlier. Adjusted earnings per share were $0.58, above the $0.51 consensus, although CFO John Rogers said the adjusted result declined from a year earlier because the prior period benefited from discrete tax items.
The company’s filing said Medical Products & Therapies sales rose 6% year over year, helped by its Spectrum IQ infusion pump, digital solutions, administration sets and international pharmacy compounding. The filing said lower injectable sales partly offset those gains. Advanced Surgery sales rose 12%, principally on higher global volumes of hemostats and sealants, while Healthcare Systems & Technologies sales rose 4%.
Baxter reported diluted earnings per share of $0.57, compared with −$0.09 a year earlier. Its filing said the prior-year result included special items that reduced earnings by $0.78 per diluted share. Gross margin was 33.4%, compared with 33.5% a year earlier, while operating margin rose to 7.56% from 6.07%. Baxter reported free cash flow of $448.0 million, compared with $126.0 million a year earlier; Rogers said the quarter benefited from the timing of cash receipts, including a tariff refund and legal settlement proceeds.

What management said on the call
CEO Andrew Hider said market demand was generally steady, with Advanced Surgery, drug compounding and infusion systems among the quarter’s largest growth contributors. CFO John Rogers said drug compounding demand reflected health systems seeking outsourced services, while injectable sales declined because of price deflation and manufacturing challenges at Round Lake. Rogers said Baxter had not observed a change in U.S. hospital capital spending but was monitoring inflation and interest-rate pressures on investment decisions.
Rogers said the adjusted gross margin decline reflected sales mix and a lower contribution from pricing. Asked about pricing, investor relations representative Kevin Moran said the comparison reflected the prior-year benefit from group purchasing organization renewals. Rogers said cost actions and higher volumes helped offset mix pressures in the Healthcare Systems & Technologies segment.
Hider said Baxter would discontinue cephalosporin production at Round Lake and reposition the facility for other injectable products. Asked about the decision, he said it would simplify operations, support supply resiliency and help address some issues referenced in an FDA warning letter. He said, “Product quality, patient safety, and supply reliability are top priorities.” Rogers said Baxter would continue to support contracted customers into 2027.
On competition, Hider said Advanced Surgery had gained volume while a competitor faced a supply challenge, creating a more difficult comparison ahead. For the fourth quarter, Moran said Baxter expected continued growth in Advanced Surgery and drug compounding, although perhaps not at the same pace.
Rogers said, “Reducing leverage remains our top near-term capital allocation priority.” Asked about capital allocation, he said he and Hider would examine investment in research and manufacturing alongside acquisitions and possible share repurchases. For 2027, Rogers identified inflation and the end of Vantive transition services as factors, with the latter’s impact depending on how quickly Baxter removes stranded costs. Hider said Baxter was examining offsets to the cephalosporin exit but was not quantifying them on the call.
Guidance
Baxter raised its 2026 organic sales growth outlook to 3% to 4% from 2% to 3%, and its reported sales growth outlook to 4% to 5% from 3% to 4%. Hider said Baxter also raised the lower end of its adjusted earnings outlook to $2.05 to $2.15 per diluted share, from $1.95 to $2.15.
Rogers said Baxter now expects low- to mid-single-digit organic sales growth in Medical Products & Therapies, compared with its previous low-single-digit outlook. He also said expected transition-services income and other Vantive-related reimbursements had risen to $165.0 million to $175.0 million from $155 million to $165 million.
